No enrollment is necessary for Behavioral Mental Health assistance once you are enrolled in a Health Insurance Plan.

Pre-Certification
Pre-certification/continued stay review is a program designed to help you and your dependents avoid unnecessary or excessively long hospital stays.
Submit a Claim
Mental health claim forms can be obtained from the Aetna website at
www.aetna.com/docfind/custom/emory. When completed, claim forms should be sent to:

If you receive services from an in-network provider, you do not have to submit a claim as the provider will submit the claim on your behalf.
