Basic and Supplemental Life Insurance
Upon retirement, you continue to receive the Basic Life Insurance at no cost to you. If you retire as a regular full-time employee, you receive $10,000 basic life insurance coverage. If you retire as a regular part-time employee working at least 20 hours or more, you receive $5,000 basic life insurance coverage.
For Supplemental Life Insurance, upon retirement, you may continue coverage for yourself and a covered spouse if you are under age 70. You must have supplemental coverage in order for your spouse to have coverage. Coverage terminates when you reach age 70, however, the policy can be converted to an individual policy at that time.
Portability of Supplemental Life Insurance
At retirement you may continue your Supplemental Life Insurance on yourself and covered spouse if you are under age 70. Portability of your spouse’s coverage is only possible if you have supplemental coverage. The policy provisions remain the same with the exception of the coverage terminating when you reach age 70. Requests for portability must be submitted to The Standard within 31 days of retirement.
To port your coverage, first obtain a Supplemental Life Coverage Continuation Request form from the Benefits Department. You will be sent the form along with a copy of your enrollment form(s) attached. You need to complete the form and return all enrollment form copies to The Standard. Submit these forms and your first quarterly premium to the Standard within 31 days of the date of last premium paid. You will be billed quarterly for premium due. A billing charge will be added to each quarterly bill.
Conversion of Supplemental Life Insurance
The conversion option permits you, a covered spouse, and eligible dependents to convert coverage to an individual policy. Conversion must be applied for within 31 days after termination of employment. Evidence of insurability is not required. To convert coverage, request a Life Conversion Information Request form from the Benefits Department. Complete the employee section and mail to The Standard. You will then be sent a description of the conversion plan, premium rates and an application form. For additional information, contact The Standard at (866) 756-8118.
