SUBMITTING REIMBURSEMENT REQUESTS


To receive reimbursement, all eligible expenses must be incurred while you are covered under the plan(s). There are three methods available for using your FSA:
  


 arrow   METHOD 1 – USE THE DEBIT CARD (AETNA/PAYFLEX CARD)


Healthcare FSA

When you enroll in the Healthcare FSA, you will automatically receive the Aetna/PayFlex debit card in the mail.

NOTE: The use of the debit card is for convenience only. IRS guidelines still require you to retain receipts for any eligible expense that the card is used for. Aetna may request verificatin of expenses and you will need to submit the appropriate documentation.



Dependent Day Care FSA

When you enroll in the Dependent Day Care FSA, you will automatically receive the Aetna/PayFlex debit card in the mail.

If you have both a Healthcare FSA and a Dependent Day Care FSA, you will receive one card and both balances will be on the one card, however, Healthcare and Dependent Day Care FSAs are separate and cannot be combined. In other words, you cannot request reimbursement of unused Healthcare FSA dollars to cover Dependent Day Care expenses, and vice-versa.

 

Aetna Limited FSA (for HSA Plan members): Debit cards are not available for the Limited FSA. You must file a claim form to PayFlex for reimbursement.


 


arrow   METHOD 2 – FILE A CLAIM ONLINE

You can submit a claim online and then upload, mail or fax your documentation along with a fax cover sheet. For assistance creating your online profile, click here.




arrow  METHOD 3 - SUBMIT A PAPER CLAIM

You can download a claim form and mail or fax the completed claim form along with your documentation to Aetna (instructions are on the form).

Healthcare FSA: For eligible healthcare expenses, you must first submit the claim to your medical or dental carrier. Once the claim is processed and you receive the Explanation of Benefit (EOB) from your medical or dental carrier, submit a copy of the EOB and a claim form to Aetna/PayFlex for reimbursement.

Dependent Day Care FSA: For eligible dependent care expenses, pay the provider directly and then submit the receipt and an a claim form to Aetna/PayFlex for reimbursement.

Limited FSA (for HSA Plan members): For eligible health care expenses, you must first submit the claim to your medical or dental carrier. Once the claim is processed and you receive the Explanation of Benefit (EOB) from your medical or dental carrier, submit a copy of the EOB and a claim form to Aetna/PayFlex for reimbursement.
