LIFE INSURANCE



arrow  Basic Life Insurance

Emory Clinic provides eligible physician members, professional members, senior/founding members and key executives with basic life insurance in the amount of one times salary up to $50,000. There is no cost and enrollment is automatic. All life insurance benefits are administered by The Standard Insurance Company.



arrow  Supplemental Life Insurance

Emory Clinic also provides supplemental life insurance in the amount of $450,000 of coverage. Enrollment is automatic.



arrow  Voluntary Supplemental Life Insurance

You can elect voluntary supplemental life insurance coverage in increments of $50,000, up to a maximum guaranteed issue of $250,000 at the time of your initial eligibility without Evidence of Insurability (EOI). Coverage in excess of $250,000 up to $500,000 will require EOI. Premiums will be deducted from your salary monthly. If you initially waived coverage, EOI is required.



arrow  Voluntary Supplemental Life Insurance for Spouse/Same Sex Domestic Partner (SSDP)

You can elect voluntary supplemental life insurance coverage for your spouse/SSDP in the amount of $4,000 as a rider on your Supplemental Life Insurance coverage. Additional voluntary supplemental life insurance coverage for your spouse/SSDP is available in increments of $50,000, up to a maximum guaranteed issue of $50,000. Coverage in excess of $50,000 up to $500,000 will require EOI. Premiums for life insurance coverage on your spouse/SSDP will be deducted from your monthly salary. If you initially waived coverage, EOI is required.

The amount of dependent life insurance for your spouse/SSDP may not exceed $100% of the amount of your voluntary supplemental life insurance coverage.



arrow  Voluntary Supplemental Life Insurance for Child(ren)

You can elect voluntary supplemental life insurance for your eligible child(ren) in the amount of $4,000 as a rider on your supplemental life insurance coverage. Additional voluntary life insurance coverage for your child(ren) in increments of $10,000, up to a maximum of $30,000.

The amount of dependent life insurance for your spouse/SSDP may not exceed $100% of the amount of your voluntary supplemental life insurance coverage.

Coverage begins on the date of hire or upon approval of Evidence of Insurability (EOI) if required.



arrow  Your Beneficiary

You should designate a beneficiary to receive your life insurance benefits upon your death. Beneficiaries can be one person or multiple people, charitable institutions, or your estate. Once named, your beneficiary remains on file until you change it. If your family situation changes, review the beneficiaries on file and make updates if necessary.

If you do not name a beneficiary, your life insurance benefits will be paid in equal shares to the first surviving class of the classes below.

arrow  Reductions in Insurance

At age 70, life insurance coverage for you and your spouse/SSDP reduces to 65% of the original face amount; at age 75 it reduces to 65% of the amount of insurance coverage in effect immediately prior to age 75.
