Update Name or Gender
Employees are required to notify Data Services of changes in address, phone number, emergency contact information or change of name or gender. Departmental Human Resources Representatives are required to notify Data Services of any other changes to employment or job information for an employee.
How to Change Your Name Information
University employees should notify Data Services as soon as possible after a name change. To change a name in a Human Resources record, the following documents must be completed and signed by the employee:
- Personal Information Change Form
- Updated Form I-9
which verifies identity and employment eligibility as required by the Federal government.
- Contact your department Human Resources Representative to complete the name change section, Section III, of your original I-9 form, OR
- Complete a new I-9 (all three sections) with your department Human Resources Representative or a member of the Data Services staff. If you complete a new form, fill out Section I with your OLD name and Section III with your new name.
- New Tax Forms: W-4
(Federal withholding) & G-4 (Georgia withholding)
- New Social Security Card (original only) or a receipt from the Social Security Administration
The Payroll Department cannot accept copies or faxes of tax forms or direct deposit forms.
Data Services cannot accept copies or faxes of Form 1-9.
Social Security Card
According to the Social Security Administration, if you need to change the name on your Social Security card, you must show proof of your legal name change. The Social Security Administration accepts these documents:
- marriage document
- divorce decree specifically stating you may change your name
- certificate of naturalization showing a new name
- court order for a name change
If the documents provided for a legal name change do not provide sufficient information to identify you (date of birth or age), you must also show two identity documents. These two documents must contain your date of birth, age, or a recent photograph.
- one document with your old name
- a second document with your new name
All documents must be either originals or copies certified by the issuing agency. Photocopies and notarized copies will not be accepted.

How to Change Your Gender Information
University employees should notify Data Services as soon as possible after a gender change by completing a Personal Information Change Form . If the employee name is also changing, the Name Change procedure must also be followed.
