Emory University partners with Equifax to offer you the opportunity to get your tax forms electronically. This convenient service allows you to receive your tax forms much quicker. 2017 tax forms include the W-2 form as well as the 1095-C form which serves as documentation of your eligibility for health care coverage.

In order to receive your tax forms online, you must log on to Self-Service and consent to receive them electronically NO LATER THAN 5 p.m., Thursday, January 11, 2018. For step-by-step instructions, click here.

If you have previously consented to receive your tax forms online, you do not need to do it again.

If you do not consent to receive your forms online, your 2017 tax forms will be mailed to your home address. Please verify that your home address is listed correctly in Self-Service by clicking on the Personal Information tile.
