The Kaiser Permanente health plan operates as an integrated care model. This means all care must be coordinated through Kaiser providers, within Kaiser facilities, and managed by your designated Kaiser Primary Care Provider (PCP). While services may differ in structure, Kaiser members have access to many of the same resources as those enrolled in the Aetna plan, delivered through Kaiser’s coordinated care system.

To begin your weight management journey, contact your Kaiser PCP. They will assess your needs and initiate a referral to the appropriate program.
Available Support Options
Depending on your care plan, your PCP may refer you to:
- A nutritionist for personalized dietary counseling
- Online tools and digital education resources
- Access to self-guided programs through Kaiser’s member portal
Program Update
Kaiser’s TransforMe in-person program, which previously included an out-of-pocket fee, has been postponed until further notice due to staffing shortages.
