Guidehealth®

Guidehealth provides Emory University and Emory Healthcare employees and their enrolled dependents, who are enrolled in certain Emory Health Plans, with personalized health support. Whether you have recently been to the hospital, are managing a long-term condition, or simply need help navigating your healthcare, the Guidehealth team is here to help.

What is Guidehealth?

Guidehealth is Emory Health Plan’s care management platform. Their team of nurses and care coordinators work alongside your existing care team to help you get the right care, at the right time. Guidehealth does not replace your care team and does not provide medical advice or clinical treatment, rather they support and complement your existing care plan.

Guidehealth has years of experience working with clinicians within the Emory Health Plan network.

Who is Eligible?

You are eligible if you are enrolled in one of the following Emory Health Plans:

  • Aetna POS (Point of Service)
  • Aetna HSA

Covered dependents enrolled under your plan are also eligible. If you are enrolled in the Kaiser Health Plan, this program is not available to you.

What Does it Cost?

There is no copay, no deductible, and no insurance claims filed when you use the service.

How Do You Get Started?

If you are eligible to participate in this program, Guidehealth will reach out to you if you have recently been hospitalized, visited the emergency room, or if you are managing a long-term condition.

You can also contact the Guidehealth care team at any time for support with:

  • Connecting with a nurse care manager
  • Getting help before or after a procedure
  • Managing chronic conditions within the care management setting (e.g. diabetes, high blood pressure, heart or kidney disease, or dementia)
  • Maternity or high-risk pregnancy care coordination
  • Navigating pediatric care needs or maternal medicine
  • Finding an in-network primary care provider or assistance with scheduling an appointment
  • General questions about navigating your healthcare

No. Guidehealth and Aetna are two separate organizations.

Participation is completely optional. You can engage as much or as little as you like, and you may stop at any time. Your medical benefits are not impacted based on participation in the Guidehealth program.

Just like the Emory Health Plan and your provider care team, Guidehealth is also subject to state and federal privacy laws, like HIPAA.

No. Guidehealth works with your existing providers, not in place of them. Their role is to help coordinate and manage your care.

Yes. Any dependents enrolled with you in an Emory Aetna POS or HSA Health Plan are eligible for all Guidehealth services at no cost.

Guidehealth care management services are provided through the Emory Health Plan’s care management relationship with Guidehealth. Guidehealth tailors its programs to individual needs; outreach, timing, and enrollment are based on eligibility.
