New Structure for Medical Plan Rates
In 2025, Emory introduced a new approach for medical plan rates, where the amount you pay will be higher or lower depending on how much you are paid. The reason for this is because the cost of the medical plan is more of an employee’s income if they are lower paid, and we want to ensure all our employees have access to affordable health care.
There are three salary-based rate bands to determine employee contributions:

Tobacco Use Surcharge
To support the health and well-being of our faculty and staff, Emory has implemented a $50 per person monthly tobacco use surcharge on medical plan contributions for employees and their spouses who are covered on the Emory medical plan and use tobacco products.
When you enroll, you must certify online in Self-Service whether or not you and your spouse have used tobacco within the last 60 days. The per person tobacco use surcharge will be waived if:
- You certify that you and/or your spouse have not used tobacco within the last 60 days; OR
- A physician is currently treating you for a medical condition such as nicotine addiction. In this case, you and your physician will need to complete and return the Tobacco Cessation Physician Affidavit form to the Benefits Department.
